To the best of the breeders
knowledge, the puppy is in good health and has received all vaccinations and worming as to the age of the puppy. The puppy
was examined by _____________ and deemed to be in good health.
The puppy is guaranteed to be free of life-threatening
diseases at the time of adoption and free of life-threatening congenital defects for one year. Should the puppy be diagnosed
with a life-threatening congenital defect, the puppy will be replaced at no charge as availability permits or a refund of
the adoption fee. A returned puppy must have a letter from a licensed veterinarian stating the reason for return along with
the registration papers. The breeder reserves the right to obtain a second opinion.
A licensed vet check must be done
within 7 days of the date of adoption, at the expense of the adopting parent for puppy's protection or the above guarantee
is null and void. LEPTOSPIROSIS vaccines can be deadly to a doxie of any age. If puppy is given a leptospirosis vaccine,
it will null and void the guarantee. Any veterinarian costs incurred by the adoptee after picking up the puppy are solely
the adopter's responsibility.
If the adopting parent is unable, for any reason and at any time, to keep the puppy, the
breeder will gladly accept it back at any age with the registration papers. The adoptee agrees that UNDER NO CIRCUMSTANCES
will the dog/puppy ever be turned over to a shelter or abandoned. If you cannot continue to provide the puppy with a home,
the puppy/dog is to be returned to the breeder. Refund of adoption fee is not guaranteed except as previously mentioned above.
We cannot guarantee adult size, conformation, color, fertility, or trainability. We do not charge show dog price, and do
not guarantee that you are buying a show dog.

The puppy should be neutered at the age determined by your veterinarian for puppy's protection & health.
The puppies has been raised on __________________ and if you decide to change food please do so gradually to prevent
any upset to the puppy's stomach. This can cause diarrhea which can lead to dehydration and hypoglycemia. The adopting parent
agrees to care for the puppy humanely and provide a loving, safe and comfortable home with an extra emphasis on the TLC.
